Colgate invites qualified dental hygiene professionals to apply for the honor of participating in the Colgate Oral Health Advisory Board Program (the "Program"). The selected participants will serve on the Colgate Oral Health Advisory Board. To be eligible for selection, applicants must:
- Be state licensed as a dental hygienist in the United States and be actively employed as such as of the date of application;
- Have a minimum of ten (10) patients indicate recognition of hygienist's contribution to patients' oral health by statements on the consumer Colgate Oral Health Advisor website
- Recommend at home oral care products to patients;
- Not work for or consult with any oral care products company other than Colgate, formally or informally;
- Must be available (if selected) to attend Advisory Board business meeting at Colgate Palmolive headquarters in New York City and Technology Center in New Jersey from 11/10 through 11/12/10 and, thereafter, to participate in Advisory Board Meetings (dates and number to be determined).
- Colgate may choose to communicate with selected group at least 2 more times during the year to ask their opinions as needed.
How to Apply:
To apply, dental hygienists must enroll and fill in all of the required information. Enrollment is limited and may be discontinued at Colgate's discretion. Only entries that comply with these terms and conditions will be considered.
Patients must recognize their dental hygienists by August 15, 2010 in order for the hygienist to be eligible for selection for the 2010 Colgate Oral Health Advisory Board. Each dental hygienist may only be recognized once per patient.
Seven (7) applicants (one from each of 7 geographic regions) will be selected by Colgate and offered the opportunity to serve on the Advisory Board. The evaluation process will be based on the information provided online, the patient's qualitative comments when referring the dental hygienist and a telephone interview by one or more Colgate representatives. The seven selected applicants will be invited to attend a three day/three night orientation and business meeting in New York, NY to take place 11/10-11/12/10. Trip will include air and ground transportation (if applicable), hotel accommodations (one single occupancy room and room tax only) for two nights, two dinners, a trip to the Colgate Technology Center and attendance at three Advisory Board meetings, expenses paid by Colgate. All other expenses not specifically mentioned herein are the responsibility of the participant.
Successful applicants will be expected to provide Colgate with written opinions on dental hygiene issues at least twice during the following year. All decisions of Colgate and its representatives will be final. No correspondence will be entered into with applicants who are not selected via the process outlined above. Applicants assume all liability for any injury or damage caused by, or claimed to be caused by, participation in this offer. Applicant agrees to release, discharge and hold harmless Colgate, its respective directors, employees and agents from and against any and all liability arising out of participation in any activities relating to this offer.
Selected participants consent to allow Colgate to use of his/her name or image in any material related to this Program.
Selected participant will be required to sign a confidentiality agreement.
Participants selected to the Advisory Board will be notified via e-mail and announced on the Colgate Oral Health Advisor Website, www.colgateoralhealthadvisor.com.
